INFORMATION FOR PATIENTS

Thank you for choosing us for your vein treatment. This page is intended to help new patients become better acquainted with our office. If you have a question that is not answered here, we encourage you to contact us and we’ll be happy to help.

APPOINTMENTS

We are available to schedule appointments Monday through Friday from 8 – 5. We kindly ask that you give at least 24 hours notice if you are unable to keep your scheduled appointment with us.

PRESCRIPTION REFILLS

Prescription refills should be handled during business hours Monday through Friday. We require 72 hours notice when you need your prescription refilled. Be prepared to provide us with your pharmacy phone number when you call.

PRIVACY POLICY

We take the privacy of your personal health information seriously. You will be provided with our privacy policy upon registration. We comply with all HIPAA guidelines.

ACCEPTED INSURANCE

We accept most health insurance plans including Medicare.

PAYMENT FOR SERVICES

You will be asked to pay for any portion of your bill that is not covered by your insurance such as co-payments, deductibles and non-covered services. We accept cash, checks and major credit cards.
